Muscimol chemical structure

Muskimol; juga dikenal sebagai agarin, panterina, atau asam piroibotenat; adalah agonis reseptor GABAA dengan efek sedatif dan halusinogenik serta konstituen psikoaktif utama dari jamur Amanita seperti Amanita muscaria dan Amanita pantherina.[1][2][3][4][5] Ini merupakan alkaloid 3-hidroksiisoksazola dan secara struktural terkait erat dengan neurotransmiter asam aminobutirat gamma (GABA).[6][7][8] Senyawa ini banyak digunakan sebagai ligan dan agonis reseptor GABAA dalam penelitian ilmiah.[9][10] Muscimol biasanya digunakan secara oral (termasuk juga diisap dalam bentuk rokok).[11][12][13][14] Efek puncak terjadi setelah 1 hingga 3 jam secara oral[15][16][17] dan durasinya adalah 4 hingga 8 jam tetapi hingga 24 jam.[18][19][20]

Efek muskimol pada manusia antara lain meliputi depresi sentral, sedasi, tidur, gangguan kognitif dan motorik, halusinasi, distorsi persepsi, dan kedutan otot.[21][22][23][24] Muscimol bertindak sebagai agonis penuh reseptor GABAA yang poten.[25][26][27] Ia juga merupakan agonis parsial reseptor GABAA-ρ yang poten dan penghambat penyerapan kembali GABA yang lemah.[28][29][30] Obat ini tidak aktif pada reseptor GABAB tetapi merupakan substrat dari GABA transaminase (GABA-T).[31][32][33] Muskimol sebagian besar memberikan efeknya melalui aktivasi reseptor GABAA.[34] Muskimol sangat berbeda dari obat-obatan seperti benzodiazepin dan barbiturat karena merupakan agonis ortosterik reseptor GABAA dan bukan modulator alosterik.[35][36] Tidak seperti GABA, muskimol menembus sawar darah otak dan karenanya aktif secara sentral.[37][38][39] Muskimol, yang juga dikenal secara kimia sebagai 5-aminometilisoksazol-3-ol, adalah analog GABA yang terkendali secara konformasi.[40][41] Senyawa terkait dan konstituen Amanita spp. asam ibotenat adalah bakal obat dari muskimol.[42][43][44]

Muskimol pertama kali diisolasi dari Amanita muscaria dan karenanya ditemukan pada tahun 1964.[45][46][47] Telah dilakukan studi klinis terbatas sebagai obat farmasi potensial untuk sejumlah penggunaan seperti pengobatan epilepsi.[48][49][50][51] Selain itu analog dan turunan muskimol, seperti agonis reseptor GABAA selektif gaboksadol (THIP, LU-2-030) dan penghambat penyerapan kembali GABA selektif tiagabin, telah dikembangkan sebagai obat farmasi.[52][53][54][55] Muskimol dan jamur Amanita muscaria jarang digunakan sebagai obat rekreasional secara historis.[56] Namun, pada pertengahan tahun 2020-an, penggunaan zat-zat ini termasuk penggunaan rekreasional untuk efek halusinogenik dan mikrodosis untuk manfaat terapeutik yang diklaim, menjadi semakin menonjol.[57][58][59][60] Alasan terapeutik yang paling sering dikutip untuk penggunaannya adalah untuk meningkatkan kualitas tidur.[61] Muskimol bukanlah zat yang dikendalikan dan tidak diatur di sebagian besar dunia, termasuk di sebagian besar Amerika Serikat dan Eropa.[62][63][64]

Sumber alami

Sumber alami utama muscimol adalah jamur dari genus Amanita seperti Amanita muscaria dan Amanita pantherina. Senyawa ini diproduksi dalam jamur bersama dengan muskarin (yang terdapat dalam jumlah sedikit dan tidak aktif), muskazona, dan asam ibotenat.[65][66] Pada Amanita muscaria, lapisan tepat di bawah kulit tudung mengandung jumlah muscimol tertinggi, dan oleh karena itu merupakan bagian yang paling psikoaktif.[67]

Penggunaan dan efek

Sifat dan efek muskimol pada manusia telah dinilai secara terbatas dalam studi klinis.[68][69][70][71] Dalam studi ini, muskimol dinilai pada dosis 5 hingga 15 mg secara oral. Dosis ambang oral muskimol kira-kira 6 mg,[72][73] sedangkan kisaran dosis psikoaktif dikatakan berkisar antara 8 hingga 15 mg.[74][75][76] Hanya 1 g "kancing" Amanita muscaria kering dapat mengandung jumlah muskimol ini, meskipun potensinya sangat bervariasi di antara jamur.[77] Menurut Jonathan Ott, dosis 15 mg bersifat "psikoptik" sedangkan dosis 20 mg bersifat "visioner".[78] Awitan kerja muskimol, melalui konsumsi muskimol atau Amanita muscaria secara oral, terjadi antara 30 menit dan 2 jam,[79][80] dengan efek puncak terjadi setelah 1 hingga 3 jam. Durasi efeknya adalah 4 hingga 8 jam, tetapi beberapa efek dapat bertahan hingga 24 jam.[81][82][83][84][85][86] Dalam satu publikasi, efek muskimol dijelaskan sebagai berikut:[87][88]

"Waser (1967) menjelaskan efek pemberian muskimol sendiri sebanyak 10–15 mg sebagai '. . . halusinasi intens seperti pada LSD tidak ada . . . terjadi gangguan fungsi psikis yang cukup besar seperti orientasi ruang dan waktu, persepsi visual, proses berpikir, bicara, dan beberapa fenomena psikis baru berupa ilusi dan gambar gema'. Dosis yang lebih tinggi cenderung menghasilkan intoksikasi parah pada manusia, dengan kedutan otot yang menyakitkan, agitasi yang cukup besar, dan halusinasi yang jelas".[89][90]

Efek muskimol pada manusia dalam berbagai penelitian telah ditemukan meliputi sedasi, pusing, inkoordinasi, relaksasi, pengurangan kecemasan, perbaikan suasana hati, tidur, mimpi yang beragam, kesulitan berbicara, gangguan perhatian, fokus, dan konsentrasi. Efek samping muskimol meliputi gangguan belajar, kebingungan, kehilangan nafsu makan, stimulasi, agitasi psikomotor, efek halusinogenik, pseudohalusinasi seperti gema (penglihatan dan suara), halusinasi yang jelas, psikosis, dan delirium.[91][92][93][94] Pada dosis yang lebih tinggi, koma, sawan, dan kematian dapat terjadi. Efek fisik muskimol di antaranya dapat meliputi kedutan otot, kemerahan, sedikit peningkatan tekanan darah, mual, muntah, sakit perut, dan diare.[95][96][97][98] Efek samping yang dilaporkan meliputi kelelahan, kurang aktif, sakit kepala, dan migrain.[99] Muskimol dapat meningkatkan kadar prolaktin dan hormon pertumbuhan.[100][101]

Muskimol dikatakan memiliki efek serupa pada tidur pada hewan pengerat seperti obat farmasi eksperimental terkait gaboksadol (THIP).[102][103][104][105] Pada manusia, gaboksadol menurunkan latensi awal tidur, meningkatkan durasi tidur, meningkatkan tidur gelombang lambat (SWS) dan aktivitas gelombang lambat (SWA), serta tidak menekan tidur REM.[106] Efek muskimol dan gaboksadol pada tidur berbeda dari modulator alosterik positif reseptor GABAA yang banyak digunakan seperti benzodiazepin dan obat Z, yang justru dapat mengganggu SWS dan SWA meskipun meningkatkan awal dan durasi tidur.[107][108][109][110] Meskipun muskimol dan gaboksadol memiliki efek serupa pada tidur, muskimol juga ditemukan dapat meningkatkan tidur REM, tidak seperti gaboksadol.[111]

Asam ibotenat selaku bakal obat muskimol, aktif pada dosis sekitar 20 hingga 100 mg secara oral pada manusia.[112][113][114][115] Sekitar 10 hingga 20% asam ibotenat dikatakan diubah menjadi muskimol setelah dekarboksilasi.[116][117]

Toksisitas dan overdosis

Institut Federal Jerman untuk Penilaian Risiko memperingatkan bahwa muscimol dan produk yang mengandungnya menimbulkan risiko kesehatan yang serius, terutama bagi anak-anak.[118]

Profil toksisitas dan keamanan muscimol telah dipelajari dalam berbagai konteks, baik eksperimental maupun klinis. Median dosis letal (LD50) pada mencit adalah 3,8 mg/kg s.c. LD50 pada tikus adalah 2,5 mg/kg i.p., dan 45 mg/kg secara oral.[119] Sebuah studi pada primata menunjukkan bahwa ketika diberikan dalam dosis yang meningkat, muscimol menyebabkan hiperkinesia dan diskinesia reversibel pada dosis yang lebih tinggi (hingga 88,8 mM), tetapi tidak ada toksisitas jangka panjang yang diamati pada pemeriksaan histologis.[120] Muscimol telah menunjukkan potensi sebagai antikonvulsan, memblokir sawan yang diinduksi oleh berbagai agen pada model hewan tanpa menyebabkan toksisitas yang signifikan pada dosis terapeutik.[121] Muscimol menunjukkan efek yang bergantung pada dosis dengan dosis yang lebih tinggi menyebabkan gejala sistem saraf pusat yang signifikan, tetapi reversibel.[122] Dosis muscimol yang dianggap berpotensi fatal pada manusia adalah 90 mg, yang merupakan 15 kali lipat dari dosis aktif ambang batasnya yaitu 6 mg.[123] Menurut Asosiasi Mikologi Amerika Utara, "dalam manusia, tidak ada kasus kematian yang terdokumentasi secara andal akibat racun dalam jamur ini (jamur yang mengandung turunan isoksazola) dalam 100 tahun terakhir, meskipun ada satu kasus di mana seorang pekemah membeku hingga meninggal saat dalam keadaan koma".[124]

Interaksi

Aksi dan efek muscimol dapat diperkuat oleh benzodiazepin seperti diazepam.[125][126]

Farmakologi

Farmakodinamika

Muscimol adalah agonis penuh reseptor GABAA yang poten, mengaktifkan reseptor untuk neurotransmiter penghambat utama otak yakni asam aminobutirat gamma (GABA). Muscimol berikatan dengan situs yang sama pada kompleks reseptor GABAA seperti GABA itu sendiri, tidak seperti obat GABAergik lainnya seperti barbiturat, benzodiazepin, dan nonbenzodiazepin, yang berinteraksi dengan situs alosterik yang terpisah.[127][128] Reseptor GABAA tersebar luas di otak, sehingga ketika muscimol diberikan, ia mengubah aktivitas neuron di beberapa wilayah termasuk korteks otak besar, hipokampus, dan otak kecil. Dengan meniru GABA, muscimol mengaktifkan reseptor ini, yang menyebabkan pembukaan saluran klorida dan hiperpolarisasi neuron selanjutnya. Hal ini mengakibatkan penurunan eksitabilitas neuron, yang sangat penting untuk menjaga keseimbangan antara eksitasi dan penghambatan di sistem saraf pusat.[129]

Muscimol menunjukkan efek yang relatif seragam pada reseptor GABAA dengan komposisi subunit yang berbeda. Namun, ditemukan bahwa muscimol bertindak sebagai superagonis reseptor GABAA ekstrasinaptik yang mengandung subunit α4β3δ (Emax = 120 hingga 140% relatif terhadap GABA).[130][131] Hal ini ditemukan disebabkan oleh penurunan desensitisasi reseptor dengan muscimol dibandingkan dengan GABA.[132] Penelitian selanjutnya menemukan bahwa muscimol merupakan agonis preferensial dari populasi reseptor GABAA yang mengandung subunit δ yang relatif kecil dan bahwa reseptor ini memberikan kontribusi substansial terhadap efeknya.[133][134] Berbeda dengan muscimol, benzodiazepin dan obat-obatan golongan Z tidak mengaktifkan reseptor GABAA yang mengandung subunit δ.[135] Di sisi lain, alkohol diketahui secara selektif mempotensiasi reseptor GABAA ekstrasinaptik yang mengandung subunit δ, analog dengan muscimol.[136][137][138]

Meskipun muscimol sering dianggap sebagai agonis GABAA selektif dengan afinitas yang sangat tinggi terhadap reseptor GABAA yang mengandung subunit δ,[139][140][141] ia juga merupakan agonis parsial poten dari reseptor GABAA-ρ, sehingga rentang efeknya dihasilkan dari aksi gabungan pada lebih dari satu subtipe reseptor GABAA.[142] Faktanya, muscimol lebih poten sebagai agonis parsial reseptor GABAA-ρ daripada sebagai agonis reseptor GABAA. Muscimol dikatakan tidak aktif pada reseptor GABAB.[143][144] Namun, sebuah studi selanjutnya melaporkan bahwa muscimol mungkin memiliki aktivitas penghambat yang dimediasi reseptor GABAB, meskipun penelitian lebih lanjut diperlukan untuk mengkarakterisasi aktivitas ini lebih lanjut.[145][146] Muscimol tidak aktif dalam hal mempengaruhi GABA transaminase (GABA-T).[147] Terdapat sedikit bukti bahwa muscimol berinteraksi dengan target biologis lain selain reseptor GABA dan transporter GABA.[148]

Pada hewan, muscimol menghasilkan depresi sentral, sedasi, analgesia, anksiolitik, efek antikonvulsan, efek neuroprotektif, dan anestesi.[149][150] Dalam studi diskriminasi obat pada hewan pengerat, muscimol dan gaboksadol sepenuhnya tergeneralisasi satu sama lain, tetapi generalisasi antara benzodiazepin seperti diazepam tidak terjadi.[151][152][153] Temuan ini menunjukkan bahwa muscimol dan gaboksadol memiliki efek interoseptif yang berbeda dari benzodiazepin.[154][155] Selama pengujian yang melibatkan kelinci yang terhubung ke EEG, muscimol menunjukkan jejak EEG yang tersinkronisasi secara jelas. Hal ini sangat berbeda dengan psikedelik serotonergik seperti psilosibin, yang pola gelombang otaknya umumnya menunjukkan desinkronisasi. Pada dosis yang lebih tinggi (2 mg/kg melalui IV), EEG akan menunjukkan lonjakan karakteristik.[156]

Farmakokinetika

Farmakokinetika muscimol pada manusia telah dipelajari secara sangat terbatas. Parameter farmakokinetika seperti bioavailabilitas, volume distribusi, pengikatan protein plasma, dan waktu paruh eliminasi tidak tersedia.[157][158]

Absorpsi

Muscimol mudah diserap di saluran pencernaan ketika digunakan secara oral.[159]

Distribusi

Distribusi jaringan otak muscimol pada tikus telah dipelajari. Muscimol dengan cepat masuk dan terdistribusi tidak merata di otak tikus, terutama di substantia nigra, kolikuli, dan hipotalamus.[160][161] Muscimol menembus sawar darah otak dan karenanya aktif secara sentral.[162][163][164] Hal ini dikatakan kemungkinan dimediasi oleh transpor aktif melalui sistem penyerapan GABA afinitas tinggi dan transporter asam amino lainnya.[165][166][167] Meskipun muscimol menembus sawar darah otak, ia melakukannya dengan relatif buruk dan jauh lebih sulit daripada gaboksadol.[168][169]

Metabolisme

Muscimol diketahui dimetabolisme melalui transaminasi oleh GABA transaminase (GABA-T).[170][171][172] Asam ibotenat merupakan bakal obat muscimol melalui dekarboksilasi.[173][174][175] Namun, telah dikatakan bahwa muscimol juga dapat diubah kembali menjadi asam ibotenat melalui glutamat dekarboksilase.[176] Metabolit muscimol belum diidentifikasi, tetapi mungkin berkontribusi pada toksisitas muscimol.[177]

Eliminasi

Muscimol diekskresikan oleh ginjal ke dalam urin. Diekskresikan sebagian dalam keadaan tidak termetabolisme.[178] Hal ini telah dimanfaatkan oleh praktisi Siberia dalam penggunaan enteogenik tradisional Amanita muscaria melalui daur ulang muscimol dalam urin.[179]

Waktu paruh eliminasi muscimol pada manusia tidak diketahui.[180][181] Obat yang terkait erat dengannya yakni gaboksadol (THIP), yang merupakan turunan siklik dari muscimol, memiliki waktu paruh eliminasi pada manusia selama 1,5 hingga 2 jam.[182] Pada hewan pengerat, waktu paruh gaboksadol sekitar dua kali lebih lama daripada muscimol.[183] Obat ini dikatakan lebih tahan terhadap metabolisme daripada muscimol, misalnya tidak menjadi substrat untuk GABA-T.[184][185]

Kimia

Struktur

Muscimol pertama kali diisolasi dari Amanita pantherina oleh Onda pada tahun 1964,[186] dan dianggap sebagai asam amino atau peptida. Strukturnya kemudian diuraikan oleh Takemoto,[187] Eugster,[188] dan Bowden.[189] Muscimol adalah isoksazola semi-kaku yang mengandung substituen alkohol dan aminometil.[190] Muscimol umumnya digambarkan sebagai tautomer, di mana ia mengadopsi konfigurasi seperti amida.[191] Ia juga umumnya ditampilkan sebagai ion zwitter.[192]

Sifat

Muscimol adalah ion zwitter pada pH fisiologis.[193] Nilai log P yang diprediksi adalah –1,4 hingga –2,2.[194][195] Nilai log P obat ini mirip dengan asam aminobutirat gamma (GABA).[196]

Isolasi

Muscimol dapat diekstraksi dari daging Amanita muscaria dengan perlakuan air mendidih, diikuti pendinginan cepat, dan perlakuan lebih lanjut dengan resin basa. Amanita muscaria dicuci dengan air, dan dielusi dengan asam asetat menggunakan kromatografi kolom. Eluat dikeringkan beku, dilarutkan dalam air, dan dilewatkan melalui kolom selulosa fosfat.[197] Elusi selanjutnya dengan amonium hidroksida dan rekristalisasi dari alkohol menghasilkan muscimol murni.[198]

Dalam kasus di mana muscimol murni tidak diperlukan seperti penggunaan rekreasi atau spiritual, ekstrak kasar sering disiapkan dengan merebus Amanita muscaria kering dalam air selama 30 menit.[199]

Analog

Analog muscimol meliputi asam aminobutirat gamma (GABA), asam ibotenat, dihidromuscimol, tiomuscimol, asam piperidina-4-sulfonat (P4S), gaboksadol (THIP), 4-AHP, 4-PIOL, asam nipekotat, asam isonipekotat, guvacine, isoguvacine, THPO, dan tiagabin.[208][209][210][211] Persyaratan struktural untuk pengikatan dan aktivasi reseptor GABAA sangat ketat, sehingga relatif sedikit agonis reseptor GABAA dengan efikasi tinggi yang diketahui.[212][213]

Sejarah

Amanita muscaria telah digunakan oleh manusia sebagai obat psikoaktif sejak zaman kuno.[214][215][216] Muscimol diisolasi dari Amanita muscaria secara independen oleh tiga kelompok penelitian berbeda pada tahun 1964 dan 1965.[217][218][219] Senyawa ini disintesis oleh Gagneux dan rekan-rekannya pada tahun 1965.[220][221][222][223] Struktur kimia muscimol, bersama dengan asam ibotenat, dipublikasikan oleh Conrad Eugster di Universitas Zürich pada tahun 1967.[224][225][226]

Kesamaan strukturnya dengan neurotransmiter asam aminobutirat gamma (GABA) dengan cepat dikenali, dan muscimol terbukti memiliki aksi seperti GABA oleh Graham Johnston dan rekan-rekannya pada tahun 1968.[227][228] Selanjutnya, aksinya terbukti dapat dibalik oleh antagonis reseptor GABA bikukulina pada tahun 1971.[229][230]

Efek muscimol pada manusia dipelajari dan dijelaskan oleh Waser pada tahun 1967.[231][232][233] Kemudian, seorang ahli etnobotani yakni Jonathan Ott lebih lanjut menjelaskan efek muscimol, melalui konsumsi Amanita pantherina, pada tahun 1976.[234][235]

Ahli kimia medisinal asal Denmark yakni Povl Krogsgaard-Larsen dan rekannya mempelajari muscimol dan analog sintetisnya selama beberapa dekade mulai tahun 1970-an. Ligan reseptor GABAA lainnya seperti gaboksadol (THIP) dan 4-PIOL, dan modulator transporter GABA seperti asam nipekotat dan tiagabin, telah diturunkan dari muscimol. Banyak analog muscimol dikembangkan oleh Krogsgaard-Larsen dan rekannya.[236][237][238]

Muscimol ditemukan secara daring sebagai obat perancang baru pada tahun 2023.[239]

Masyarakat dan budaya

Status hukum

Muscimol bukanlah zat yang dikendalikan dan tidak diatur di sebagian besar dunia.[240][241][242]

Australia

Muscimol dianggap sebagai zat terlarang "Jadwal 9" di Australia berdasarkan Standar Racun (Oktober 2015). Zat Jadwal 9 adalah zat "yang dapat disalahgunakan atau digunakan secara tidak tepat, pembuatan, kepemilikan, penjualan atau penggunaannya harus dilarang oleh hukum kecuali jika diperlukan untuk penelitian medis atau ilmiah, atau untuk tujuan analitis, pengajaran atau pelatihan dengan persetujuan Otoritas Kesehatan Persemakmuran dan/atau Negara Bagian atau Wilayah."[243]

Polandia

Muscimol dan asam ibotenat keduanya dianggap sebagai zat psikoaktif baru di Polandia pada tahun 2024. Memiliki atau menjualnya adalah ilegal.[244]

Amerika Serikat

Baik Amanita muscaria maupun muscimol tidak dianggap sebagai zat yang dikendalikan oleh Pemerintah Federal Amerika Serikat. Badan Pengawas Obat dan Makanan Amerika Serikat (FDA) telah menyatakan Amanita muscaria dan komponennya (termasuk muscimol) tidak disetujui untuk makanan konvensional dan juga sedang mengevaluasi penggunaannya dalam suplemen makanan.[245] Regulator pertanian di Florida mengambil tindakan terhadap seorang penjual produk Amanita setelah badan tersebut menentukan bahwa produk tersebut dianggap tercemar menurut hukum negara bagian.[246]

Muscimol dapat diatur di tingkat negara bagian. Undang-Undang Negara Bagian Louisiana 159 melarang kepemilikan dan budidaya Amanita muscaria kecuali untuk tujuan hias atau estetika. Undang-undang tersebut melarang sediaan Amanita muscaria yang ditujukan untuk konsumsi manusia (termasuk muscimol), Kecuali sebagai bagian dari makanan atau suplemen makanan yang diproduksi secara sah.[247]

Penelitian

Muscimol telah dipelajari secara klinis untuk sejumlah potensi penggunaan medis.[248][249][250][251] Obat ini dinilai dalam studi klinis kecil dalam pengobatan penyakit Huntington, diskinesia tardif, dan skizofrenia pada tahun 1970-an tetapi tidak ditemukan bermanfaat untuk indikasi ini.[252][253][254][255][256][257][258][259] Studi lain mengevaluasi muscimol pada penderita skizofrenia dengan diskinesia tardif pada tahun 1992.[260] Studi juga menilai efek biokimia muscimol pada manusia pada akhir tahun 1970-an dan awal 1980-an.[261][262][263]

Menurut Povl Krogsgaard-Larsen, muscimol terlalu toksik dan tidak selektif sehingga tidak dikembangkan untuk digunakan sebagai obat farmasi.[264][265][266] Sebagai gantinya, dikembangkanlah analog sintetis gaboksadol (THIP) yang lebih selektif dan kurang toksik.[267][268][269]

Pada tahun 2019, uji klinis fase 1 muscimol untuk epilepsi resisten obat dipublikasikan.[270][271] Muscimol juga telah secara resmi diteliti untuk potensi pengobatan penyakit Alzheimer dan penyakit Parkinson.[272][273] Sebuah tinjauan sistematis dan metaanalisis tahun 2023 dari 22 studi praklinis menemukan bahwa muscimol mengurangi nyeri neuropatik pada hewan, dengan efek yang dimulai dalam 15 menit dan berlangsung hingga 3 jam.[274][275]

Muscimol belum pernah disetujui sebagai obat farmasi untuk penggunaan apa pun di mana pun di dunia.[276][277]
